What will the petition responses be used for? Will they be reported to agencies or insurance groups? The privacy notice at the bottom of the page...

completely confidential and will never be shared."...seems to be in conflict with compiling "required" names & addresses on a petition. If only the number of responders will be reported to others, then why collect the extra data? Will petition signers be put on your commercial email list?
Also, do you have a reference for "15% of Americans with TRD commit suicide"? Wow, that's a lot.
A trivial question: How is the phrase "VNS Therapy" a trademark? Isn't that like Pfizer claiming a trademark on "drug therapy"?
