Well, if you're hallucinating badly, I don't suppose it occurs to you. But if you're seeing things that probably aren't too realistic to see, you might consider the possibility.
Once when I was as close to delusional as I ever want to be (thank you Psychobabble), I walked in my house and glanced in the kitchen. There was a huge rat looking back at me. I honestly wasn't sure it was real. Fortunately the exterminator found evidence that he had been there, and heard him in the walls. I never thought I'd be so happy to know we had a rat in the kitchen.
