Treatment-Resistant Dysthymia

Hello Everyone,

I've suffered from Dysthymia for about 7 years now and have had very little, to no, success with medications. I've long thought it to be Atypical Depression but it seems more like a chronic moderate depression with double episodes. I had limited responses from Lamictal, Bupropion, and Lexapro/Bupropion combinations. I've recently dived into Atypical Antipsychotics and have had glimpses of success. Abilify restored my sex drive and seemed to improve my circadian rhythm but made me too tired. I'm not on Risperdal and it is helping my circadian rhythm but that's about it thus far. I'm also on EMSAM 6mg and don't really think it is doing anything.

I believe my 'success' with the AP is the fact that it tingles the 5-HT7 receptors, thought to improve depression, sleep, and thermo-regulation. Other than that, I still suffer from daytime sleepiness and overall fatigue. I'm treated for Sleep Apnea and have had tons of blood tests done to rule out other things so I'm unsure what could help me. I've tried lots of stimulants and atypical stimulants (Provigil/Nuvigil) and had some good days, some bad days, and some irritable days - I could not predict the reactions.

Thoughts on where to go next?
