Garlic is high sulfur. For some reason, certain people feel poorly from sulfur foods and certain people really need a lot of it. Sulfur has a big role in the body's detox system, since many toxins bond (weakly) to the sulfur molecule. People suspected of being toxic from amalgams should probably avoid sulfur as it stirs up more mercury than it removes, making things worse.
Garlic is also a strong critter killer...bacteria, fungi, viruses. Maybe someone feeling better with garlic is lowering the burden of hidden unknown infectious micro-organisms?
And though I am not sure on this one, I believe garlic is anti-inflammatory, which would give it a good role in pain, fatigue, or psychiatry.
Anyway, yeah, I have heard of people feeling noticeably better with garlic, and also people feeling noticeably worse.
