Sulpiride and amisulpride are "clean" drugs compared to most other antipsychotics, in the sense that they don't grope on all sorts of receptors. That means, for example, that they are not antihistamines, so they are not sedating and don't stimulate appetite.

They are more liable to increase prolactin than other antipsychotics except for risperidone. This is because they cross the blood-brain-barrier poorly, meaning higher peripheral concentrations are required to reach the desired central effects. The pituitary gland, which produces prolactin, is peripheral, and reduced dopamine agonism translates to higher prolactin.

Elevated prolactin is a factor that can lower sex steroids.
