What about sulpiride? One possible factor for social phobia is that our brains may be stuck with fearful responses to social stimuli because of past trauma, but we're unable to destroy these responses and learn new ones even when the trauma is long past. Several studies with lab rats show that sulpiride enhances fear extinction in lab rats. I don't know how it does this and I don't know nearly enough about this stuff but it might have to do with its action on D2 and D3 dopamine receptors or maybe some neurogenerative properties. I'll make a new post about that down at the bottom. Anyway although it's classed as an AP, sulpiride acts as a stimulant and an AD at non-neuroleptic doses (50mg-200mg) because of the way it binds to dopamine receptors. It also has anxiolytic properties at doses closer to 200mg. It's very cheap and easy to get outside the U.S.
tepi do the thoughts come only when you're in public or are they there all the time? Are they like obsessive and intrusive? Do you ever feel when you're outside or even when alone like you're sort of disconnected from your body? Having anxiety for a long time leads to other stuff developing, like dissociation.
I have the same problems like you but I seem to be getting a therapeutic response at 200mg sulpiride. You could ask your doc to let you try the same meds I'm on: 300mg amitriptyline + 200mg sulpiride. You could try 50mg sulpiride for a few days and see how you feel as it has a rapid onset of action.
