I think akathisia is a sign that the antipsychotic dose is too high ie. excessive dopamine antagonism is present. Akathisia is very distressing and can lead to an exacerbation of psychosis. Benzodiazepines such as diazepam and anticholinergics such as diphenhydramine can be used temporarily.
