I would not combine a SSRI with moclobemide. The danger of inducing serotonin syndrome seems very real. I'm not sure why this combination was ever deemed safe. I don't understand why the reversibility of the MAO inhibition produced by moclobemide would prevent it from inducing a hyperserotonergic state as do the irreversible MAOIs when used this way. I know Joffe and Bakish have combined them. I am surprised they haven't reported having any problems when doing so.
