Putting aside the ethics of marketing, the problem is that even with fiercely competitive strategies, there isn't much scientific progress going on, but rather a reliance on cloning formulas from 30 year old prototypes. Even the the drugs from the 50s are now being compared favourably and they certainly didn't sink millions into those at that time.
