milnacipran has something closer to a one to one or a one to three ratio of NE to 5HT, whereas cybalta is similar to effexor with a much greater ratio of serotonin to norepinephrine reuptake inhibition.
i don't know why milnacipran isn't in the states, it's very popular in japan due to its lack of sexual side effects. its also thought that it has a lessened "discontinuation syndrome".
its fast acting, and quite safe, and has applications for neuropathic pain, why we have to order over seas is due to FDA or patent approval/cost/profit margin.
most people i've heard prefer milnacipran; cymbalta seems to have more side effects.
