Since seroquel has been linked to tardive dyskinesia, we know that it isn't all that aytpical after all, so even with all the serotonin antagonism and what not what you're really looking at is a souped-up Thorazine.
Its akathisia, most likely, as you say -- but akathisia is still rare on Seroquel (as I can attest that only Zyprexa and Seroquel do not give me akathisia and I am sensitive -- of course, needless to say, everyone is different) and even more rare on Clozaril (other unpleasant side effects on the other hand are more and harder to manage with Clozaril). Also TD, while some reports have occurred, is also generally more rare on Seroquel than say, Risperdal. One journal report put a statistical average about 2% for TD across the board for atypicals. Another I've seen with a larger number of patients is about 1/2% annually for Zyprexa (British Journal of Psychiatry 174: 23-30 (1999). Its not like Haldol by any means, especially EPS, since EPS and TD are somewhat on a continuum.-- Jay
