Tricyclic ADs seem to be very good for panic attacks--you may wish to try one such as nortriptyline, which does not have particularly severe side effects. It works much more powerfully on norepinephrine than Effexor does and may be a better choice for your depression than Effexor, which is still skewed towards serotonin even at the relatively high dose you are taking. There may be other meds worth trying also, but in any event, it sounds like you might benefit from some kind of change.
