It's a strategy that some psychiatrists have used with success. This seems to be something that the MAOIs are more amenable to.

You ask the right questions. Unfortunately, I don't have the answers. One of my doctors had a patient whom he switched between Parnate and Nardil regularly when poop-out occurred. In this way, he didn't have to go without treatment.

It might yet return. Many people experience episodes of medication breakthrough despite overall stabilization. Perhaps your recent changes of medication have perturbed your system enough to stress it so as to cause this to happen.
The tramadol idea sounds interesting. With a short half-life of 6-8 hours, it might be somewhat safe to work with in case of the emergence of an adverse event like SS. By the way, the only TCA I deem to be absolutely safe to combine with Nardil is desipramine.
