Kramer: why do meds ''poop out''/stop working?

Can't find a thread where this has already been asked (which surprises me!) so here it is - can you please explain why after a period of successful use, a med will suddenly "poop out"/stop working, no longer work to treat the condition it was effectively treating? Thanks!
