Just my own ideas but I would ask your pdoc to try one of the other SSRIs or SNRIs instead of Paxil or Effexor. Both have very short half-lives & discontinuation symptoms are pretty horrible if you want off them. Why not try one with a longer half-life like Prozac. The shorter the half-life the more intense the discontinuation symptoms. Prozac seems to be one of the most tolerable to stop when you no longer need it.

I've found it a real bugger to get off Paxil & after two attempts, I'm taking a low dose to stop the discontinuation symptoms till summer, when I'm going to try again.
