Dr. Shipko, of the Panic Disorder Institute (www.algy.com/pdi/BBS/) has mentioned using low-dose hydrocone in a no. of his treatment-resistant patients. You can e-mail him. Something about possible opiod receptor dysfunction.

There is a piece on the unexpected benefits of Ultram in Dr Bob's tips. There seems to be a small sub-group of patients who only respond to opiates. Like Steve says, this population is enriched with OCD sufferers.
