There's a lot of controversy over any AD in bipolar disorder. I think the "switch" rate for Effexor is lower than for tricyclics, but I'm not really sure. I also don't know how good the data for that is (a lot of psychiatric wisdom is based on low-quality data).If there's a lot of mood instability, agitation, psychosis, aggression, paranoia, etc., I wouldn't think an activating AD like Effexor would be a good choice. Then again, I'm not a shrink. Most people with a bipolar diagnosis spend more time in depression than any other mood state, so AD prescriptions are fairly routine.

I'm bipolar and I take Effexor. It may destabilize your mood, but you can always treat that with antipsychotics or additional mood stabilizer.
