I've been doing some searches on combining antipsychotics when I came across a little fact that something like >90% of people prescribed antipsychotics in Japan are on at least 2.
Here in N.America, there are trials beginning to emerge, and it looks like the combination of either clozapine + risperdal or olanzapine + risperdal have some kind of promise. Nothing really *amazing*, but I will say from my subjective experience, combining olanzapine and risperdal has rescued me in very difficult times. Well I take 2 APs. Zyprexa and perphenazine. It seems to work realy well! Zyprexa alone was not enough after the abilify. I added perphenazine and didn't need that realy high dose of zyprexa anymore. Still I'm taking more AP than before my abilify experience. I'm sleeping more. But I seem to be doing well now.

I don't know if this is really a new thing. I mean, using atypicals+atypicals or atypicals+conventional neuroleptics is probably new, but mixing neuroleptics has been done for a long time...haldol+thorazine, neulactil+whatever, so on and perphenazine+whatever.
I read somewhere that antipsychotic polypharmacy seems to result in increased costs and more side effects. I wish I could link to that article, but I didn't bookmark it or anything. That said...it seems to me (as a non-professional) that careful use of 2 atypicals, especially if one is low-dosed and/or just used PRN, might be useful...like when people have zyprexa or something as a "rescue med" for freak-outs.
Do you have any info on why they do it so often in other countries? Are the people on antipsychotics there regarded as more severe cases than the people here? Do they not use benzos as much?

They actually use tonnes of benzos in Japan, but fewer antidepressants.
