From MAOI to MAOI: Washout period required?

Posted by bearfan on July 27, 2010, at 15:08:10

I am currently taking Moclobemide (RIMA), and since it seems to have a low rate of success, if i'm unhappy with it, how long would I wait until I try something like Parnate? It doesn't seem to me that it would require a 2 week waiting period, since MAO is already deactivated and by the 2nd day the system is washed out from Moclobemide. Same question applies to Emsam to Nardil or Parnate to Nardil.

After stopping moclobemide, you only need to wait a few days before starting another antidepressant. 2 week washout periods are normally recommended if switching between Nardil and Parnate.

I'm still trying to understand the science behind why a 2 week period would be necessary. It makes sense that say a SSRI user who wants to start a MAOI, they have to wait up to 4-5 half lifes before starting to avoid the risk of serotonin system. It also makes sense that after a MAOI, the user must wait 2 week before then can start a sertonergic drug because the MAO enzyme take about 2 weeks to reactivate. But if it's MAOI to MAOI (which generally have short half lives), why the 2 week waiting period? MAO would still deactivated.

I think it's based on reports of serious reactions which have occured when switching from Nardil to Parnate and vice versa.

I don't understand the reasoning, but when I went from lexapro to ensam, I had to washout for two weeks, and when I went from ensam to parnate, I had to wait 10 days.

Your doctor probably wanted to avoid serotonin syndrome.
