Paradoxical reaction from modafinil

Posted by tensor on April 4, 2010, at 7:57:07

I have had a hiatus from modafinil for about a year. I tried modafinil (200mg) a week ago and I actually got something done, but it didn't work as it once did. Since then I only get tired from it, so tired I have to sleep. If I take 200mg in the morning, or even 300mg, I will sleep for several hours as soon as it kicks in.
Why? I had the same reaction to bupropion, it worked wonderfully (as in 100% remission) for a few weeks, then suddenly I just wanted to sleep when I took it.
It's like I'm running out of dopamine or something. Ideas greatly appreciated.

Hi Mattias,

Sounds a bit like my reaction to coffee. When I feel tired during the day, coffee often makes me more tired and I need to sleep. If I drink coffee at night, I still feel tired but it sometimes disturbs my sleep. Whatever the caffeine was doing it's clearly not what I need. I guess the same applies to modafinil for you.

Hi Ed,

I wonder if Dexedrine would poop-out in the same way. I, who once said to never consider antipsychotics, are realizing they are the next logical step (and maybe have been for a while). The spring is coming along with a million things that have to be done, so I feel something more drastic has to been done, so Abilify is next.

Just a side note I've had times when coffee tired me greatly or could drink a pot and nothing, then it switched and panic? Strange is this chemistry changes or what? Phillipa

A long time ago, I think it made me nervous. It doesn't have that effect anymore.

The effects of antipsychotics can be very dose-dependent, so you have to be careful how much you take. Will you start with 5mg Abilify? We don't have the 2mg tablets here, I don't know whether you do. I think it would be best to try half a 5mg tablet to start with. That's what I'd do anyway.

Also, get the doctor to prescribe you a few tablets of an anticholinergic (eg. procyclidine or similar) in case you have a dystonic reaction or akathisia.

The smallest here are 5mg too. I was thinking of 2.5mg/day or perhaps every other day, to start with. And yes, I have a pill cutter :-)

Sounds like you're ready to start :)

I can make no sense of my reactions to stimulants, caffeine, and xanax. They are never consistent in their effect. It seems like for the most part stimulants and coffee don't do much, and I could sleep right after them. Or sometimes I wonder if they are making me tired.
A small dose of xanax doesn't do much either, except on rare occassion it will knock me out.

I'm the same as you. Bupropion worked great for me for about 2-3 months and then it did nothing. Provigil/Nuvigil would work some days, would do nothing another day, and make me tired and irritable the other day. Same things happen with coffee and other stimulants.

Not sure what to really try anymore either, as I'm in desperate need of energy and motivation.

Interesting. Are you considering an atypical AP as well? I read up on Remeron's mechanisms and it should boost dopamine, I have never been above 60mg and maybe it's worth a shot. I have no problems waiting a little with Abilify, so to speak :-)

I've been on a few Atypical Antipsychotics (AAP) - Risperdal/Invega and Abilify. I don't think I gave Abilify as long as I should but Risperdal/Invega gave me horrible side-effects: sensitivity to light, retrograde ejaculation, and bouts of panic and constant worrying.

I think the problem with AAPs and me is that they antagonize dopamine receptors when really what I need is a specific agonist. Dopamine antagonists tend to make you sleepy and more anhedonic which is what I don't need. I'd like to try Amisulpride if I can order it, or maybe one of the RLS drugs (Mirapex/Requip).

Remeron has always made me curious but the fact that it tends to be sedating and makes you eat more has always steered me away. I need more energy and need to eat less.......so it seems too risky for me.
