Waiting for EMSAM

Well, the wait is on for a number of us for EMSAM and according to the BM's recent powerpoint, there looking for a release sometime in April. The SSRI's never did much for much, except reduce what I call life force, and blank your brain. So, having been diagnosed with Social Phobia as well as depression, I'm looking forward to trying out EMSAM at the 20mg patch level. Not looking for the skies to part and euthymia to appear. Given that it took over a decade for an already approved substance to be given transdermally rather orally, hopefully this approval heralds an era of antidepressants which are known for their theraputic advantages, rather than their side effects.

I read a press release the other day from BMS which gave out 4 different email addresses to BMS and Somerset people involved with the release of EMSAM. I emailed all of them asking when it would come out.

To my surprise every one of them wrote me back. One said "mid April" and the other 3 said "towards the end of April." So, those who are waiting to try it (me included) would appear to have 3 to 4 weeks to go.

Since I'm a total non responder to SSRI's, SNRI's and a dozen other meds for social anxiety and depression I'm really looking forward to my first shot at an MAOI particularly with reduced concerns of food restrictions and what is supposed to be minimized side effects (we'll see about that).

I'll post my results once I get started.

There is no 20 mg dosage. FDA only approved patches of 6, 9 and 12 mg. and only 6 mg has mo diet restrictions, 9 and 12 have the restrictions.

Do the 6, 9, and 12mg patches all have the same drug-drug interaction restrictions as the oral version of MAOIs?

When I say 20, I'm talking about active ingredient in the patch itself, of which 6mg is designed and intended to be taken up into the bloodstream over 24 hours.

Here are the different patches of mass over area, hence the 20 of which 6 mg is intended to be absorbed, 9 from 30, 12 from 40. See the fda presentation.

20mg/20cm2 (6mg)
30mg/30cm2 (9mg)
40mg/40cm2 (12mg)

HOW SUPPLIED
EMSAM (selegiline transdermal system) is supplied as 6 mg/24 hours (20 mg/20 cm2), 9 mg/24 hours
(30 mg/30 cm2) and 12 mg/24 hours (40 mg/40 cm2) transdermal systems.
They are available as:
NDC 39506-033-30: 6 mg/24 hours (20 mg/20 cm2) box of 30 transdermal systems.
NDC 39506-044-30: 9 mg/24 hours (30 mg/30 cm2) box of 30 transdermal systems.
NDC 39506-055-30: 12 mg/24 hours (40 mg/40 cm2) box of 30 transdermal systems.

From the BMS website:

EMSAM (selegiline transdermal system) can cause serious and
potentially life-threatening reactions if used with certain other
medicines. Do not take the following medicines while using EMSAM,
and for 2 weeks after stopping EMSAM:
• Other medicines to treat depression (antidepressants) including other
MAOI medicines
• Medicine which contains selegiline (such as Eldepryl®)
• St. John’s wort (an herbal supplement)
• Demerol® (meperidine), or medicines that contain meperidine (a narcotic
pain medicine) or the pain medicines tramadol, methadone, or
propoxyphene
• Tegretol® (carbamazepine), or other medicines that contain carbamazepine
(a seizure medicine)
• Trileptal® (oxcarbazepine), or other medicines that contain oxcarbazepine
(a seizure medicine)
• Cold or cough preparations that contain dextromethorphan
• Flexeril® or other medicines that contain cyclobenzaprine (a medicine
used to treat muscle spasms)
• Decongestant medicines, found in many products to treat cold symptoms
• Over-the-counter diet pills or herbal weight-loss products
• Any herbal or dietary supplement that contains tyramine
• Medicines called amphetamines, also called stimulants or “uppers”
• BuSpar® (buspirone HCl), an anxiety medicine
Some of these medicines will have to be stopped for at least a week
before you can start using EMSAM.

Thanks for posting that info. I really want to try an MAOI, but the thought of completely going off my medication cocktail for depression/anxiety scares me; I don't think I could do it.
