Provigil and psychosis?

Hi,

I've been around reading for a while but this is my first post. I have been diagnosed as schizo-affective and had my last brief psychotic episode almost two years ago. At the moment I am still suffering from negative symptoms, esp. social withdrawal, anhedonia and depression. I am currently on Abilify, 10mg. Recently, I have added 100mg of Provigil to help with daytime sleepiness and motivation. I have found this to be a good combo as I am also benefiting from the moderate improvement in mood from the Provigil.
My pdoc however, is having second thoughts about the Provigil. He is afraid that it increases the likelyhood of me becoming psychotic again. Is this fear warranted? I have not found any information on modafinil and psychosis except for one report of a healthy person suffering an episode after a 600mg dose. Any thoughts?

I am on provigil, and I have plenty of experience with neurological symptoms from hell {screwy hallucinations, etc}.

Yes, provigil can do what your doctor fears. Basically, with extra sensitive brains, rage and psychosis are potential side effects because of the effect it has on dopamine. This was an explicit warning before i took my first ever dose.

Just tell your doctor you will keep him posted.

They actually give provigil to schizos to counteract antipsychotic negative side effects. I know of an article from currentpsychiatry.com that has a detailed case report about using it with zyprexa I believe on a very sick schizo {extremely low functioning}.

Thanks for your input on this. I thought the effect of Provigil on dopamine was marginal and that it primarily triggers orexin signalling. Well, I think I'll stick with it since it is really helping a lot. Also, not falling asleep at work is probably a good idea.
I think I saw the report you mention. They reported a positive effect on functioning having added Provigil and increased Zyprexa on this patient. I just don't know how widespread the use of Provigil for negative symptoms and AP side effects really is.
