Hi. I was recently on Strattera 40 mg. and had to discontinue due to extreme drowsiness and insomnia (weird combo, huh?) so my pdoc put me on 36 mg. of Concerta (taken in the a.m of course) and I feel much more relaxed and productive. The concentration is hard to gauge though. What is a good indication that the medication is working? I am still a little spacey, but maybe I'm just giving it too much thought. The side effects I've noticed are blurry vision, mild headaches that come and go, and slight dizziness. Everyone talks about decreased appetite on stims. I actually had less of an appetite on Strattera than on this. Does anybody else not notice a difference in appetite on Concerta? That is one side effect I'd love to have, and I'm kind of disappointed I'm not experiencing it. Also, I'm supposed to be taking 10 mg. Lexapro and 200 mg. Topamax for my Bipolar, but I'm nervous because I read that there is a drug interaction for SSRI's and methyphenidate, and also with seizure meds (which is what Topamax is) should I just get over the fear and take it? I do get anxious being on so much medication. Any tips on how to overcome that? Thanks!

Hi -
Can you describe the extreme drowsiness and insomnia a bit more? I think I know what you mean - sometimes I'm just in utter stupor all day, but can't sleep at night. Really tired, but just can't get or stay asleep. Is this like what you're experiencing?
I also have an increased appetite on any type of stimulant medication. I think it's because I'm so tired and I just can't/don't want to eat, but with a stimulant, I feel awake and wanting to eat.
I don't know about the Topamax/SSRI/Ritalin combo - hopefully someone will be able to answer that.
