question for Cam or John L.

Posted by gilbert on May 23, 2001, at 22:12:24

Hi guys I figured I get out of the chicken and the egg debate and back to some legitimate med questions..I have tried prozac at 10mg, effexor at 37.5, and celexa at 10mg, all as single drug monotherapy, and at very low dosages and still got anorgasmia. Pdoc doesn,t mind me on xanax which works but I get a little depressed on it but still functional. He wants me to try zoloft at 25mg, or go back and titrate prozac at 5 mg. for a while and see if anorgasmia still appears. What do you 2 guys think is my best option. I know zoloft has some dopamine antagonist but is it enough to circumvent anorgasmia or should I see if my body can adjust to 5mg. prozac. I tried all of the above drugs for at least 3 to 4 weeks each. Weird thing on the prozac as horny as a spring bunny with no ability to finish.

Thanks,

Gil

I don't think you will find an SSRI that doesn't have sexual dysfunction as a side-effect. But adding buproprion may help.

Gil - Re: Chicken and egg. Did you ever post something and then realize that your ideas are more radical than you initially thought?

As for anorgasmia while taking SSRIs. As Sal says, all SSRIs (and many TCAs) can cause anorgasmia in suseptible people. Clinically though, I have seen a trend, but I am not sure if the science has bourne it out. It seems that the Paxil™ (paroxetine) is the worst offender (made a chemical eunich out of me). Zoloft™ (sertraline) seems to be less of an offender, but still did delay orgasm in me. Paxil doesn't seem to be as bad, but I have heard a few women complain about anorgasmia. With Effexor™ (venlafaxine) I have heard varying results. For some people it completely shuts off any thought of sex, while others say that the quality of their orgasm improved (I can attest to the latter; just call me Cannon Man - although the orgasm does appear to be a bit more delayed.

As for whether to keep using the Prozac™ (fluoxetine) or to switch to Zoloft; it's six of one and half a dozen of the other. Really, since you have started the Prozac, you may as well continue and see if the anorgasmia lessens as your body gets use to the drug. If the situation doesn't improve in 4 to 6 weeks, then change to Zoloft.

In closing, it is difficult to say how your body will respond to the different SSRIs, so you must keep up the trial and error.

Good luck - Cam

When I was still working the serotonin end of the equation and tired of the sexual side effects, I tried Wellbutrin and Serzone, neither of which has sexual side effects (reportedly and to my knowledge, anyway). They operate a bit different than the usual SSRIs.

Thank you for your input I should have added that I had tried Serzone and it still did the same thing to me plus I felt awful. I also was on Wellbutrin and it put me on the ceiling plus gave me tachycardia seems anything that bumps my norepherinine geeks my whole system. The xanax has been my saviour for 2 years but I do get tired of sleeping in etc. It may be I just can't take AD's for my panic I may be looking for something that isn't there.

Hate to ask this--sort of. Have you tried the natural precusors for panic? Choline for acytl... GABA for ? Magnesium (1000 mg/day) Taurine (1000 mg/day) For me, they don't do the job alone but they help when I'm getting almost enough support from my meds for panic. On the SSRI side, there's 5HTP, phenylalmine, glutamate, l-tyrosine, St. John's Wart & SamE.

You said something better than I could have said it myself: "Maybe I'm looking for something that isn't there." If you go here, you can read my long rambling about the same subject. I also am a sufferer of panic attacks and if you read the whole thread, you'll see we are talking about just what you mentioned in opening the subject. http://www.dr-bob.org/babble/20010522/msgs/64047.html

Hi Gil,
I think Prozac is the least likely among the SSRIs to cause sexual problems. Like Cam said, I think Paxil is the worst. I had some anorgasmia when I started Prozac, and like you it really pepped up my sex life, and the anorgasmia went away on its own in about 3 to 4 weeks. I titrated up very slowly from about 2.5mg a day to 20mg a day over a period of about a month.

The juice trick can help. Empty the Prozac capsule into a glass of juice, stir it well, and drink a custom sized dose. Save the rest in the fridge for the next dose. Hopefully the anorgasmia will diminish like mine did.
John
