I have extreme nighttime anxiety/insomnia. My doctor
is talking about Remeron. I have tried everything
else in the psychopharm universe and this seems to be
a last resort. The Neurontin was a weird, horrible
experience.I have read that Remeron is only effective for about
1 month? Is this true? What's everybody's experience?Help!

> Help!I have never been on Remeron but read about it. Its an interesting antidepressant, in so far as its mechanism of action is concerned. It blocks noradrenaline and serotonin auto-receptors which are the feedback receptors that control how much NE or 5-HT is released. By blocking these receptors they give the brain negative feedback, meaning your brain thinks there is less 5-HT and NE in the brain so fires more of these neurotransmitters. Unlike other convnetional antidepressants it does not make the supply of the monoamines available for a longer period of time. Consequence-This may burn out the supply of these neurotransmitters hence why it may only be effective for a month in some cases. There will always be options for treatment-refractory depression. If you know whats the biological root of the problem Im sure the guys and gals at this board will be able to offer you some positive suggestions.

It's been effective for my anxiety/depression, but I do find that the positive effects peak for me about 6 weeks in, then seems to drop off after that, and I often need to increase the dose. I also have been really stressed with school and taking ritalin regularly so that may play into the peaking/dropping off that I've experienced. Overall, I can't say enough about how much this med has helped turn around my life. It has given me motivation, and the ability to enjoy my life.

Here are the benefits of Remeron:
--It can be used for all types of depression, anxiety, OCD, and insomnia.
--It is convienient because it is taken only once at bedtime.
--It may improve libido and sexual performance (its the only medication that improves mine). Dr. Bob Hsieung's fellow doctors recommend it for sexual dysfunction.
--You can take Remeron with most antidepressants except MAOIs.
--It has none of the anticholinergic effects of SSRIs.
--It has gained recognition as a very useful medication from psychiatrists in only four years of FDA approval.I myself started Remeron at 45 mg right away and had no side-effects. I had no weight gain or sedation during the day.

I would try it because even with all that being said I am thinking of going back.
