I have read not to take Prozac with the cough suppressant dextromethorphan, that it could lead to serotonin syndrome. Does anyone know anything about this?? I am very sick and need some nyquil, but it has dextromethorphan in it. I just started Prozac (after a loooonnnnggg weening off of Effexor) and am not familiar with all the do's and don't's . Is it really dangerous to mix these?

LD - Dextromethorphan (DM) is primarily metabolized by a liver enzyme system (cytochrome P450-2D6 or CYP-2D6). Prozac is a potent inhibitor of this enzyme. When inhibited, there is less of this enzyme to metabolize the DM. Therefore blood levels of DM will increase and may cause hyperactivity, unsteadiness, nausea, and vomitting.
In other words, taking a normal dose of DM with Prozac is like taking a very large dose of DM at one time, therefore should be avoided.

LD - Steaming and non-drug throat lozenges. I like
Fishermans Friend Original Strong (white package) the best. Strong eucalyptus flavor, but they work. To a lesser extent Halls and Vicks work. Tea and honey is also worth a try. - Cam

Hi LD,
Aside from CAM's warning of Prozac inhibiting the liver breakdown of dextromethorphan (DM) leading to increased DM blood level and side effects, anecdotal cases of serotonin syndrome with the combined use of SSRIs (e.g., Prozac) and DM have been reported. So, avoid the SSRI-DM combination, if possible.

Add my own list of meds to the question above: Prozac, Adderall, Serzone, Klonopin, Amitriptyline, and Neurontin.
I have had a household full of sick people and I'm feeling just a little worn out being unable to take anything for this wicked bug. My kid's Doc prescribed a big bottle of Codeine laced Guiatuss AC Syrup (bottle size #480), can I take any of that? So far I'm living on those awful Fisherman's Herbal gritty lozanges.
Any advice or help will be received with huge amounts of gratitude!

Pandora - Effexor and Serzone no. Clomipramine, Elavil & Paxil maybe (a slight chance; probably not significant, though). Prozac seems to be the worst offender.
