Hi, my pdoc agreed to prescribe me Asenapine on top off abilify clonazepam and nobiten
for depression and paranoia left over negative symptoms, what is your idea on that, ive asked him myself

Hi Jaroen,
I think this is a good idea. Asenapine acts as both an antidepressant / antipsychotic. It also has neurotrophic effects, which many antipsychotics do not, so (in theory) it may could positive effects on memory / cognition over time.

Im on abilify 25 mg. Clonazepam 1 mg twice. Nobiten 5 mg

Yeah, you could certainly try it at night (start low and go slow). It has some effects as an antihistamine, but it is not as strong in this regard as amitriptyline or doxepin.
Long term, you probably won't find it super sedating.

It's your body.
I often split pills when starting a medication (to allow a slower taper). If you end up taking less than prescribed, just tell your doctor.
For example, amitriptyline comes in 10mg (smallest dose), but I increased by 5mg, as I am sensitive.

Hmm. I'm not sure why you can't split the pills.
They may say this because it is on patent, and splitting pills (i.e. buying double the dose and cutting it in half) can save people money.
Splitting may cause the drug to release faster, but it has a fairly long half life (24 hours) so, it should last a full day whether it releases slowly or more quickly.

I put sarcosine on my list of things to try after you mentioned it. I need to stabilize on Parnate first.
I did not respond to intranasal ketamine. Sarcosine's mechanisms of action include working in the opposite way of ketamine on NMDA receptors. Maybe that means that Sarcosine would be a good choice for me.
The more ideas, the better. I'm thinking of going on NAC (N-acetylcysteine). Both sarcosine and NAC treatments do not produce immediate results. A good trial of NAC can take months.Thanks.

I had immediate results with both. NAC was pretty cool and strong for 2 days then i felt like absolute crap. Sarcosine made me better within days but i had akathisia.
There are many things to try, Scott, once you are somewhat restored by Parnate!
If you run out of ideas, i have plenty.

I was taking 80 mg/day of Parnate before stopping it. I went from 80-0 in two weeks. Not smart. I've done it before, though. The only negative effects in the past were profound fatigue and a worsening of depression.
I am hoping that I can take less now and still receive equal benefit. Maybe 40 mg/day.

Do you have some benefit already? Speaking in percentage, what percentage were you at on Parnate back in the days and what percentage are you at now?
